Abstract
Background: Fluid and electrolyte are human basic needed for body's metabolism. Fulfillment of the basic needs is controlled the body organ system consisting of kidneys, lungs and gastrointestinal system. Meanwhile, the fluid balance is regulated by mechanism involving thirst, hormones such as ADH, aldesterone, prostaglandins and glucocorticoids. Objectives: to explore nursing care of fulfilling fluid and electrolyte for Mr. D, patient with Chronic Kidney Disease at Inayah Ward, PKU Muhammadiyah Hospital of Gombong and able to determine nursing process consisting of assessment, formulating diagnoses, nursing intervention, implementation, evaluation, documentation and a nursing implementation analysis. Discussion: assessment data were subjective data from the patient that his urine output was little, tea like color, 5cc volume. The main nursing diagnose based on the data was the excess of fluid volume. Intervention and implementation have been done successfully consisting of assessing fluid status, measuring vital signs, monitoring laboratory results, recording intake and output, educating the fluid restriction, calculating the fluid balance, collaborating diuretic medicine as order, preparing hemodialysis. Evaluation conducted for three days were urine output still little, clear yellow urine color, 250cc volume and -21cc fluid balance. Conclusion: nursing care of fulfilling fluid and electrolyte for Mr. D, patient with Chronic Kidney Disease at Inayah Ward, PKU Muhammadiyah Hospital have been conducted by implementing nursing process. The main nursing diagnose was the excess of fluid volume that has been resolved.
