Abstract

Background : Chronic Kidney Failure is a disease that has problems with treatment therapy where side effects often occur or there are many things that are not correct (such as drug dosage), resulting in DRP's occurring. purposes : Identifying Drug Related Problems (DRPs) in chronic kidney failure patients undergoing hemodialysis at the PKU Muhammadiyah Gombong Hospital Inpatient Installation in 2023. methods : Non-experimental observational research with retrospective data collection using descriptive methods, data collection criteria in 2023, namely chronic kidney failure patients undergoing stage 4 and 5 hemodialysis. Research Results : Out of 136 patients with chronic kidney failure, there were 221 DRPs incidents, including 104 drug interactions (47.27%), 98 untreated symptoms or indications (44.54%), 14 untreated drugs (44.54%). 6.36%), the drug dose was too low in 3 cases (1.36%), duplication of the therapeutic group or incorrect active ingredient was 1 incident (0.54%), and the drug dose was too high in 0 cases (0 .00%). Conclusion: The highest incidence of Drug Related Problems (DRPs) in patients with chronic kidney failure in the inpatient installation of PKU Muhammadiyah Gombong Hospital in 2023 is drug interactions. Recommendation: There is a need for systematic monitoring and evaluation of therapy for chronic kidney failure undergoing hemodialysis which is carried out regularly in order to overcome DRPs.
