Abstract

Background: Asthma is a chronic lung disease that causes airway narrowing, leading to symptoms such as shortness of breath, coughing, and wheezing. Ineffective airway clearance is one of the main problems experienced by asthma patients due to mucus hypersecretion and bronchospasm. Pursed lip breathing is a non-pharmacological intervention that can help improve airway clearance effectiveness; however, it has not been widely implemented in healthcare facilities, including RS PKU Muhammadiyah Gombong. Objective: This study aims to describe nursing care for ineffective airway clearance in asthma patients at the Emergency Department of RS PKU Muhammadiyah Gombong and analyze the effectiveness of the pursed lip breathing technique in addressing the nursing problem of ineffective airway clearance. Method: This study employed a descriptive case study method with a nursing care approach. The research subjects were three asthma patients experiencing ineffective airway clearance at the ED of RS PKU Muhammadiyah Gombong. Data were collected through assessment, intervention, implementation, and nursing evaluation, as well as observations of vital signs and respiratory effectiveness before and after the pursed lip breathing intervention. Results: After the application of the pursed-lip breathing technique, a significant clinical improvement was observed, including a decrease in respiratory rate (from 26–30 breaths per minute to 20–24 breaths per minute), an increase in oxygen saturation (from 86%–90% to 96%–98%), and a reduction in symptoms such as ineffective coughing, crackles, and wheezing. Conclusion: The nursing care for ineffective airway clearance provided to asthma patients at the ED of RS PKU Muhammadiyah Gombong demonstrates that the pursed lip breathing technique can enhance patients’ respiratory function. Therefore, this technique can be incorporated into standard nursing interventions for acute asthma management. Recommendation: Further education is needed for healthcare professionals and patients regarding the benefits and application of pursed lip breathing. Additionally, further research with a larger sample size is necessary to strengthen these findings
