Abstract

Background: Diabetes mellitus is a chronic disease or metabolic disorder caused by insufficient insulin action or abnormal insulin secretion, characterized by increased blood glucose levels. The global prevalence of diabetes in 2022 reached 537 million people and is predicted to increase to 783 million by 2045. An imbalance in insulin production in the pancreas can lead to unstable blood glucose levels. Regular treatment, both pharmacological and non-pharmacological, can help control blood glucose levels. One effective non-pharmacological method is walking exercise therapy. Objective: To describe nursing care for blood glucose level instability in diabetes mellitus patients in the working area of Sempor 1 Public Health Center. Method: This scientific paper had used a descriptive method with a case study approach. The data had been obtained through observation of GDS measurements and interviews. The participants were three clients with diabetes mellitus who had received an innovative nursing intervention in the form of walking exercise. Results: The assessment revealed similar complaints among the patients, including weakness, frequent hunger, thirst, and elevated random blood glucose levels. The identified nursing problem was blood glucose instability. The intervention implemented was hyperglycemia management through walking exercise therapy for three consecutive days. The evaluation results demonstrated a significant reduction in random blood glucose levels. Client I showed a decrease of 55 mg/dl, Client II by 53 mg/dl, and Client III by 27 mg/dl. Conclusion: Walking exercise therapy helps reduce blood glucose levels and improve circulation in diabetes mellitus patients, thereby preventing neuropathic symptoms such as tingling, numbness, and pain in the feet and hands. Recommendation: Clients are advised to continue walking exercise therapy independently at home to reduce and control blood glucose levels.
