Abstract

Background: Chronic Obstructive Pulmonary Disease (COPD) is a progressive respiratory disorder caused by continuous exposure to cigarette smoke or pollutants that damage the lung’s cleaning system. This condition triggers excessive mucus production, obstructs the airways, and leads to coughing, shortness of breath, and respiratory distress. One of the common nursing problems encountered is ineffective airway clearance. The Pursed Lip Breathing (PLB) technique in a forward-leaning position is beneficial in improving oxygen saturation, lowering heart rate, reducing dyspnea, and decreasing anxiety and tension caused by breathing difficulties. Objective: To describe nursing care for a COPD patient with ineffective airway clearance using the forward-leaning position and Pursed Lip Breathing (PLB) therapy in the Emergency Department of RSUD Hj. Anna Lasmanah Banjarnegara. Method: This scientific paper uses a descriptive case study approach involving three patients diagnosed with ineffective airway clearance. Interventions were carried out over a period of 1x6 hours, including assessment, intervention, implementation, and evaluation of PLB therapy and the forward-leaning position. Results: There was an increase in oxygen saturation and a decrease in respiratory rate in all patients after the interventions. PLB therapy and the forward-leaning position proved effective in helping to clear the airways, reduce dyspnea, and improve ventilation. Recommendation: This study concludes that these non-pharmacological approaches can be useful and easily applied intervention strategies in emergency room settings.
