Abstract

Background: Hypertension is commonly present in haemodialysis patients and requires regular treatment. Persistent non-compliance may lead to a decline in long-term prognosis, including reduced life expectancy and lower quality of life. Patients may experience a more rapid deterioration of kidney function, potentially accelerating the need for kidney transplantation or more intensive dialysis treatment. Objective: To analyze the relationship between adherence to antihypertensive medication and the quality of life in haemodialysis patients at PKU Muhammadiyah Gombong Hospital. Methods: This study used a descriptive correlational design with prospective data collection. A total of 96 chronic kidney disease patients undergoing haemodialysis with a history of hypertension were selected during the period of April to May 2025. Data were collected using the MMAS-8 and WHOQOL-BREF questionnaires and analysed descriptively. The Gamma correlation test was used to determine the relationship between adherence and quality of life. Results: Levels of adherence to antihypertensive medication among patients varied, with most falling into the moderate category (average score: 6.53). Overall quality of life was also moderate, with an average score of 63.69. A significant relationship was found between medication adherence and all domains of quality of life (p<0.05), with the strongest correlation in the mental health domain. Conclusion: There is a significant relationship between medication adherence and quality of life; higher adherence is associated with better quality of life in haemodialysis patients. Recommendation: Further prospective studies are recommended to identify factors influencing adherence and quality of life.
