Abstract

Background: HIV/AIDS infection has become a global health challenge, with a significant increase in cases in Indonesia, including Kebumen Regency. Antiretroviral therapy (ART) is the primary management approach to suppress viral replication and improve quality of life, with viral load serving as a key indicator of success. This study aims to analyze the effectiveness of the combined ARV on the viral load of HIV/AIDS patients at RSUD Dr. Soedirman Kebumen. Methods: This study employed a retrospective cohort design, analyzing 88 out of 403 active HIV patient medical records at Dr. Soedirman Kebumen Hospital as of June 2025. The existing data will be analyzed using Chi-Square and Odds Ratio (OR) tests to examine the relationship between drug regimens and treatment effectiveness. Results: The majority of patients were male (72.72%), under 50 years old (54.54%), had a stable body weight (39.77%), and had been on treatment for at least 12 months (100%). The use of TLD and TLE regimens was balanced (50% each). Before treatment, 100% of patients had a high viral load (≥ 10,000 copies/ml). However, after treatment, 38.63% achieved an undetectable viral load. There was a significant association (p=0.025) between ARV regimen and treatment effectiveness, with patients receiving TLD being 2.833 times more likely to achieve viral load suppression compared to TLE. Conclusion: The use of the ART regimen TLD was significantly more effective than TLE in achieving viral load suppression in HIV patients.
