Abstract
Background: Patients with diabetes mellitus and ulcers are prone to psychological problems, one of which is situational low self-esteem (HDRS) due to physical changes experienced as a result of wounds, loss of limbs, loss of function, poor coping, and feelings of helplessness. Positive affirmation therapy is effective in overcoming HDRS. This therapy guides patients to think positively and recognize their remaining positive abilities, combined with butterfly hugs to increase patient comfort. Main Objective: To analyze the nursing care of diabetes mellitus patients with situational low self-esteem through butterfly hug therapy and positive affirmation at Prof. Dr. Margono Soekarjo Regional General Hospital. Method: Case study with 5 diabetic patients with low situational self-esteem with inclusion criteria experiencing ulcers > 3 years with pre/post debridement/amputation, aged 30-65 years, cooperative and willing to be respondents. Exclusion criteria included patients with impaired consciousness, mental disorders, and communication disorders. Implementation was conducted for 15-20 minutes per session over 7 meetings, starting from assessment, diagnosis, intervention, implementation, and evaluation. The instruments used were a psychosocial nursing care format, the Rosenberg Self-Esteem Scale (RSES) instrument, an observation sheet for signs and symptoms, a daily schedule sheet, the butterfly hug SOP, the positive affirmation SOP, and the psychosocial nursing care sheet. Nursing Care Outcomes: After implementation in the five patients, a decrease in symptoms was observed, with the highest decrease of 25.8% in P2, where the score before therapy was 45.1% and after therapy was 19.3%. Low self-esteem scores measured using the Rosenberg Self-Esteem Scale questionnaire also increased, with an average score before therapy of 12.2. The average score after therapy increased to 29. Therefore, it can be concluded that there was an increase in self-esteem in diabetic patients with HDRS through butterfly hug therapy and positive affirmation. Recommendations: Based on the results of this study, it is hoped that nurses and health workers can implement butterfly hug therapy and positive affirmation for patients with diabetes mellitus or other chronic diseases who experience HDRS.
