Abstract
Background: Acute Respiratory Infection (ISPA) is a common disease among children and often causes nursing problems such as Ineffective Airway Clearance. This condition is characterized by excessive secretions in the respiratory tract that interfere with ventilation. Common Cold Massage Therapy, as a non-pharmacological intervention, can help reduce secretion accumulation and improve airway patency. Objective: This study aims to analyze the implementation of nursing care for patients with ISPA who experience ineffective airway clearance through the use of common cold massage therapy at Husna Ward, PKU Muhammadiyah Gombong Hospital. Methods: A descriptive case study approach was used in this research. The subjects were five children aged 2–5 years diagnosed with ISPA and ineffective airway clearance, undergoing treatment at Husna Ward, PKU Muhammadiyah Gombong Hospital. The instruments used included a nursing care format, Nursing Kit, and Standard Operating Procedure (SOP) for innovative common cold massage interventions. Qualitative data analysis was conducted, and results were presented in both narrative and tabular forms. Results: All five patients exhibited primary complaints of cough and runny nose. Data analysis revealed additional symptoms such as tachypnea, basal rhonchi on lung auscultation, sputum production, and breathing difficulties. The main nursing diagnosis identified was Ineffective Airway Clearance. Planned interventions included monitoring breathing patterns, positioning in semi-Fowler, administering common cold massage therapy, providing family education, and collaborating with medical staff on pharmacological treatments. The intervention was implemented over three consecutive days. Evaluation results showed reduced respiratory rate, improved oxygen saturation, decreased sputum production, and better breath sounds. Conclusion: Common Cold Massage Therapy is effective in addressing ineffective airway clearance in ISPA patients by reducing respiratory rate, increasing oxygen saturation, decreasing sputum production, and improving breath sounds. Recommendation: Common Cold Massage Therapy should be integrated into standard nursing care for pediatric ISPA patients with ineffective airway clearance to enhance service quality and patient outcomes.
