Abstract
Background: Congestive Heart Failure (CHF) is a cardiovascular disease that frequently causes respiratory problems due to pulmonary congestion. This condition can lead to a primary nursing problem of ineffective breathing patterns, characterized by dyspnea, tachypnea, use of accessory respiratory muscles, and decreased oxygen saturation. The Active Cycle of Breathing Technique (ACBT) is a non-pharmacological intervention that has the potential to improve lung ventilation, assist in secretion mobilization, and enhance breathing patterns. Objective: To analyze the implementation of focused nursing care using the ACBT technique in CHF patients experiencing ineffective breathing patterns in the Emergency Department of RSUD Prof. Dr. Margono Soekarjo. Methods: This study used a descriptive case study design involving five CHF patients in the Emergency Department of RSUD Prof. Dr. Margono Soekarjo during the period of September to December 2025. The focus of nursing care included assessment, nursing diagnosis, intervention planning, ACBT implementation, and evaluation using the SOAP format. Results: The implementation of ACBT showed improvement in respiratory responses in all patients, indicated by reduced complaints of shortness of breath, decreased respiratory rate, reduced use of accessory respiratory muscles, and increased oxygen saturation after the intervention. Conclusion: The application of the Active Cycle of Breathing Technique (ACBT) as part of focused nursing care in CHF patients in the Emergency Department has a positive impact on improving breathing patterns. Recommendation: Further studies with quantitative or quasi-experimental designs and larger sample sizes are needed to evaluate the effectiveness of ACBT more comprehensively and support its application as an evidence-based nursing practice in the Emergency Department.
