Abstract

Background: Hypertension is a global health problem with a high prevalence and is a leading cause of mortality. In Indonesia, hypertension ranks fourth among the leading risk factors for death and is often accompanied by anxiety, which can worsen blood pressure and reduce patients' quality of life. Anxiety triggers a stress response that contributes to increased blood pressure, necessitating comprehensive management. Objective: To describe the application of progressive muscle relaxation (PMR) in nursing care for hypertensive patients experiencing anxiety at Prof. Dr. Margono Soekarjo Regional General Hospital, Purwokerto. Methods: This study employed a case study design. It focused on assessing and managing anxiety as a nursing problem in patients with hypertension through the implementation of nursing care interventions. Results: The results showed that all patients experienced a reduction in anxiety levels after receiving the Progressive Muscle Relaxation intervention. The Hamilton Anxiety Rating Scale (HARS) scores decreased from a range of 24–27 before the intervention to 12–14 after the intervention. Although all patients showed improvement, the degree of reduction varied depending on their physical condition and the frequency of intervention, which was conducted twice. PMR works by enhancing parasympathetic nervous system activity, thereby reducing physical tension, promoting relaxation, and calming the mind and emotions. These findings are consistent with previous studies indicating that PMR is effective in reducing anxiety from moderate to mild levels in hypertensive patients. Recommendations: Progressive Muscle Relaxation (PMR) is recommended as a nonpharmacological nursing intervention to reduce anxiety due to its effectiveness, simplicity, and safety. Routine implementation of PMR in nursing care is encouraged. Further research with a larger sample size and longer intervention duration is also recommended. Conclusion: Nursing care for hypertensive patients with anxiety in the Emergency Department of Prof. Dr. Margono Soekarjo Regional General Hospital, Purwokerto demonstrates that Progressive Muscle Relaxation therapy is effective in reducing anxiety levels from moderate to mild within 8 hours.
