Abstract
Background: Chronic Obstructive Pulmonary Disease (COPD) is a chronic respiratory disorder characterized by persistent airflow limitation. The prevalence of COPD in Indonesia reaches 3.7%, predominantly affecting males, with the primary risk factor being exposure to cigarette smoke, given that active smokers in Indonesia are estimated to number 70 million people. COPD causes skeletal muscle weakness and balance impairment, which, compounded by advanced age and chronic hypoxemia, increases the risk of falls. The family, as the primary support system, plays a vital role in fall prevention. Objective: To describe nursing care through the application of family education to address fall risk in COPD patients. Method: A descriptive case study of Mr. W (75 years old) diagnosed with COPD. The assessment encompassed vital signs, Morse Fall Scale (MFS), Mini Nutritional Assessment (MNA), Mini Mental Status Exam (MMSE), Modified Katz Index, Norton Scale, and Older Person Quality of Life (OPQOL). The family education intervention was conducted over 2 days (November 18–19, 2025) through siderail installation, education using flip chart media containing fall prevention illustrations, and demonstration of safe mobilization assistance techniques. Evaluation was conducted descriptively through observation of the family's ability to redemonstrate techniques and commitment to 24-hour supervision. Results: The initial assessment showed an MFS score of 100 (high risk), supported by the patient's age of 75 years, history of falls at home, weak gait, presence of intravenous infusion, Katz score of 7 (dependent), MNA score of 18 (at risk of malnutrition), and MMSE score of 25 (good cognitive function). First-day evaluation indicated that fall risk had not been resolved; the patient still desired to mobilize independently. Second-day evaluation showed that the MFS remained at 100 due to persistent intrinsic factors; however, the family was able to demonstrate safe mobilization assistance techniques, committed to 24-hour supervision, and planned home environment modifications. Recommendation: Hospitals are expected to standardize the use of visual educational media and actively involve families in the discharge planning of patients at high risk of falls.
