Abstract

Background: Dyspepsia is a clinical syndrome commonly experienced by older adults and is characterized by epigastric pain. Approximately 45.7% of patients with dyspepsia experience epigastric pain as their primary complaint, which may lead to the nursing problem of acute pain. Benson Relaxation, which combines deep breathing techniques with spiritual elements, is a potential non-pharmacological intervention for pain management in geriatric patients. Objective: To describe gerontological nursing care for a patient with dyspepsia and acute pain through the application of Benson Relaxation. Method: This study employed a descriptive case study design involving Mr. K, a 62-year-old patient diagnosed with dyspepsia and acute pain at UPTD Karangkobar Community Health Center. Benson Relaxation, consisting of deep breathing exercises combined with dhikr, was implemented for three consecutive days from December 22 to 24, 2025. Patient outcomes were evaluated using the Numeric Rating Scale (NRS) and vital signs assessment. Nursing Care Results: Assessment findings revealed subjective data including complaints of epigastric pain since the previous night, a burning pain sensation, nausea during meals, weakness, dizziness, decreased appetite, difficulty sleeping, and irregular breakfast habits. The pain intensity was 7/10, with PQRST characteristics: pain triggered by eating and an empty stomach (P), a burning pain sensation (Q), located in the epigastric region radiating to the lower abdomen and lower back (R), pain scale of 7 (S), and intermittent occurrence (T). Objective findings showed that the patient appeared restless, grimaced in pain, and held the epigastric area. The nursing diagnosis established was Acute Pain related to physiological injury agents. Nursing interventions included pain management and the application of Benson Relaxation for three days. Evaluation results demonstrated a reduction in pain intensity from 7/10 to 2/10. Pulse rate improved from 104 beats per minute to 63 beats per minute, and blood pressure decreased from 142/84 mmHg to 108/77 mmHg. The patient was able to perform Benson Relaxation independently, no longer exhibited pain-related grimacing, reported feeling more relaxed, experienced reduced pain, and was able to sleep comfortably Recommendation: Benson Relaxation is recommended as a standard non-pharmacological intervention for managing acute pain in older adults with dyspepsia at community health centers. The development of a Standard Operating Procedure (SOP) for its implementation is also recommended. Nurses should integrate spiritual elements according to each patient’s beliefs to optimize therapeutic outcomes.
