Abstract

Background: Continuity of Care is a midwifery care approach provided continuously from pregnancy, childbirth, newborn care, postpartum period, to family planning services. This approach aims to improve maternal and neonatal health, prevent complications, and provide optimal emotional support Objective: To implement continuity of midwifery care for Mrs. “M” G4P3A0 at 38+4 weeks of gestation until 14 days postpartum, in accordance with midwifery care standards, in order to monitor the health of the mother and baby and support maternal role adaptation. Methode: This study used a case study method with the 7-step Varney midwifery management approach. Data were collected through interviews, observations, physical examinations, and documentation studies. Care was provided directly at the patient’s home and at healthcare facilities. Result: During the period of care, the pregnancy progressed normally without complications, although the patient was initially unaware of her pregnancy and only sought antenatal care in the second trimester. The delivery occurred spontaneously at 39+6 weeks of gestation with both mother and baby in good condition. The postpartum period was physiological, and the mother chose a permanent contraceptive method (tubal ligation) after the postpartum period. Recomedation: Continuity of Care provides significant benefits in early detection of problems, increasing maternal knowledge, and achieving optimal health outcomes for both mother and baby. It is recommended that midwives continue to implement Continuity of Care as part of routine services to improve the quality of midwifery care
