Abstract
Assessment: Ny.T, aged 64 years with a diagnosis of type II diabetes when performed assessments of patients complained of pain in the heel of the right foot, continuous pain, such as heartburn, scale 7, there is a right foot wound. client said he could not sleep because noisy, the client says do not understand what food should not be eaten. Nursing Diagnosis: Acute pain associated with a physical injury agents (diabetes mellitus ulcers), disruption of sleep patterns associated with environmental impediments, lack of knowledge related to the lack of information about diet disease. Intervention: monitor pain comprehensively (PQRST), teach methods of nonpharmacological such as deep breathing and distraction relaxation, take the measurement of vital signs, collaboration analgesics, encourage clients to break on time, encourage clients improve break, give health education on diet Diabetes Mellitus , and the author also makes innovative action to reduce foot pain with techniques of hand massage. Impementation: pain comprehensively monitor (PQRST), teaches deep breathing techniques, collaboration analgesic administration, encourage clients to improve break, provide health education on diet Diabetes Mellitus. Evaluation: 1 of 3 diagnoses nursing diagnosis is still unresolved is the acute pain associated with physical injury agents.
