Abstract
Background: Chronic low self-esteem is one of the nursing diagnoses that are often found in hospitals. Clients who experience chronic low self-esteem show withdrawal behavior and avoid interacting with others. Chronic low self-esteem which is allowed to cause social isolation, hallucinations, thought process changes and the risk of violence, nursing care actions with individual psychotherapy are expected to improve chronic low self-esteem. Research purpose: Describing nursing care in patients with self-concept disorders: chronic low self_esteem in the Sampang community health center. Research method: The method of this study is descriptive with a case study approach, namely by carrying out nursing care on 3 clients of chronic low self esteem starting from the assessment, intervention, implementation and evaluation of nursing. Instrument, Format assessment, sheet observation, and SOP for mental nursing care. Result: The results of this study indicate a decrease in signs and symptoms, an increase in the patient's ability to chronic low self-esteem. The most decrease in signs and symptoms on client 1 with score of 10 (90.90%) from score 11 becomes 1, client 2 with score 9 (81.81%) from score 11 to 2, client 3 with score of 6 (50%) from score 12 becomes 6. Increase ability client the most on client 1 with score 4 (66.6%) from score 2 to 6, client 2 with score of 3 (50%) from score 2 to 5, client 3 with score 2 (33.33%) from score 2 to 4. Recommendation: Individual therapy with the Implementation Strategy (SP) approach can be applied by nurses in providing mental nursing care at the community health center services
