Abstract
Assessment: patients Br. H with a medical diagnosis of chronic renal failure when assessed says complaints of shortness of breath, coughing, nausea, lemes, defecation 2x/day slightly watery yellow, urination 2x / day the color of tea water is about 5 cc, yet knows food and beverage the recommended and banning, ascites patients, oxygen attached 4liter/min. Nursing diagnosis: ineffective breathing patterns associated with hypoventilation syndrome, nausea associated with biochemical disorders: uremia, an excess fluid volume associated with impaired regulation mechanism, deficiency of knowledge related to lack of information. Intervention and implementation: based nursing interventions classification has been done airway management, nutrition management, fluid management, and teaching: prescribed diet. The author's focus on monitoring fluid balance, as calculated between intake and output is not balanced, the patient still had excess volume of fluid (ascites) because it has not complied with the incoming fluid intake restrictions. Evaluation: evaluation after 3 days do nursing care ineffectiveness problems breathing pattern is not resolved, the problem is resolved nausea, excess fluid volume has not been resolved and the problem of deficiency of knowledge is resolved.
