Abstract

Background: Hallucinations are sensory perception disorders in which a person perceives a stimulus that is not actually there or is not real. One technique to control hallucinations is the individual implementation strategy and therapy group activity (TAK). Objective: Describe nursing care for clients with perceptual disorders: auditory and visual hallucinations with individual generalist action approaches and therapy group activity therapy (TAK) Method: Scientific paper was written using a case study method with a descriptive approach. Data obtained from interviews and observations to patients. Instruments used: observation sheets for signs and symptoms of hallucinations, observation sheets for client abilities, SP hallucinations, and group activity therapy. The number of subjects is 3 respondents and the implementation is carried out for 30 minutes each meeting for 4 days of visits Result: Perform generalist therapy and group activity therapy. The results of the evaluation of nursing care showed a decrease in signs and symptoms of hallucinations in client II from 5 to 0, while clients I and III experienced the same decrease, namely 8 to 4. There was an increase in the client's ability to control hallucinations in client II from a score of 1 to 5, and clients I and III experienced the same increase, namely 1 to 4. The most significant increase in ability to follow TAK perception stimulation occurred in client III with a score of 1 to 4, client I from 2 to 4, and client II from 3 to 4 Recommendation: The provision of generalist measures and group activity therapy (TAK) is recommended as one of the routine programs for hallucinating clients
