The innovative intervention carried out was applying head up 30˚ therapy to increase oxygen saturation to the brain (SpO2). Objective: Explaining nursing care by providing 30 head up therapy techniques for nursing problems with the risk of ineffective cerebral perfusion in the ER PKU Muhammadiyah Gombong Hospital Method: This final scientific paper uses a case study approach with a descriptive method. The instruments used are observation sheets and oximeters. Subjects consisted of 5 patients with moderate head injury with the main problem is the risk of ineffective cerebral perfusion Results: The nursing problem that the author takes is the risk of ineffective cerebral perfusion. After nursing actions for 1 x 6 hours in the ER PKU Muhammadiyah Gombong Hospital, it was found that the administration was proven to be effective in increasing the oxygen saturation of patients with moderate head injuries. Recommendation: The results of this nursing care can be used as a reference for the preparation of SOPs to increase the oxygen saturation of moderate head injury patients
