Abstract

Background: Diabetes Mellitus (DM) is a disease characterized by hyperglycemia. Type 2 DM in Central Java, especially Banyumas, reached 15,996 cases. Many type 2 Diabetes Mellitus patients are unable to maintain normal blood sugar levels. This is due to lack of physical activity, non-adherence to medication and obesity. One of the management methods used to treat diabetes mellitus is physical exercise or Progressive Muscle Relaxation (PMR) therapy, and hyperglycemia management intervention (hydrotherapy). Objective: To analyze nursing care of type 2 DM patients with instability of blood glucose level in Mawar Room at the Prof. dr. Margono Soekarjo hospital of Purwokerto. Methods: This case study involved 5 respondents with the criteria of Type 2 Diabetes Mellitus patients. The intervention provided referred to SIKI with the addition of hydrotherapy and PMR Therapy. Results: Nursing diagnosis was instability of blood glucose levels related to impaired blood glucose tolerance resulting in being tired, lethargic and high blood/urine glucose levels. Interventions: SLKI Stability of blood glucose levels (L.03022), Decreased tiredness/ lethargy, improved blood sugar levels. SIKI Management of Hyperglycemia (I. 03115) Identify possible causes of hyperglycemia, Monitor blood glucose levels, Provide oral fluid intake with hydrotherapy, Teach to do PMR therapy, Teach diabetes management, Recommend adherence to DM diet. Implementation: checking random blood sugar level, Giving the patient oral fluid intake with hydrotherapy (drinking water), Teaching the patient to do PMR therapy, Checking vital signs, and Evaluating the patients’ random blood sugar. Recommendation:, There was decrease of random blood sugar level after having hydrotherapy and PMR therapy with average of 310 mg/dL before therapy become 205 mg/dL.
