Abstract

Background: Chronic low self-esteem is one of the nursing diagnoses that are often found in hospitals. Patients who experience chronic low self-esteem show withdrawn behavior and avoid interacting with others. Chronic low self-esteem that is allowed to cause social isolation, anxiety, hallucinations, changes in thought processes and the risk of violence. Nursing care actions with individual psychotherapy are expected to improve chronic low self-esteem. Objectives: To analyze nursing care in patients with impaired self-concept: chronic low self-esteem in the Work Area of the Sempor Health Center. Methods: The method of this research is descriptive with a case study approach, namely by providing nursing care to 5 patients with chronic low self-esteem starting from assessment, intervention, implementation and evaluation of nursing. Instruments, assessment formats, observation sheets, and SOPs for psychiatric nursing actions. Result: The results of this study indicate a decrease in signs and symptoms, an increase in the ability of patients with chronic low self-esteem. Patients who experienced the most signs and symptoms of chronic low self-esteem with a score of 10 (90.90%) from a score of 11 to a score of 1 were found in patient 1 and patient 4, patient 2 with a score of 9 (81.81%) from a score of 11. to a score of 2, patient 3 and patient 5 got the same score, namely 6 (50%) from a score of 12 to a score of 6. Patients who experienced an increase in ability for patient 1 and patient 4 had the most increased ability with a score of 4 (66.67 %) from a score of 2 to a score of 6. Patient 2 experienced an increase in ability with a score of 3 (50%) from a score of 2 to 5. Patient 3 and patient 5 experienced an increase in ability with a score of 2 (33.33%) from a score of 2 increased to a score of 4. Conclusion: Individual therapy with an Implementation Strategy (SP) approach can be applied by nurses in providing mental nursing care in Community Health Center services.
