Abstract

Background: The inability of a schizophrenic client to perform self-care can lead to a client experiencing a self-care deficit. Appropriate nursing actions that have been developed in overcoming this self-care deficit, one of which is individual nursing actions (Implementation Strategy). Objective: To describe the results of nursing care in patients with self-care deficits, using "selfcare" nursing theory approach at Dosaraso Shelter, Kebumen. Methods: This paper uses a descriptive method with a case study approach. The case study instrument uses a nursing documentation format, observation sheets for recording self-care deficits, and SOPs for individual therapy implementation strategies. Subjects consisted of 5 patients with self-care deficit. The implementation of nursing is carried out 5 times and each session is 15-30 minutes long. The data were analyzed descriptively on nursing care. Results: The results of the case study showed that there was a decrease in symptoms of self-care deficit and an increase in self-care ability before and after the implementation of the strategy treatment. The results of the nursing evaluation in the five patients showed a decrease in 1-2 signs of self-care deficit symptoms, all patients were in a state of dirty and disheveled hair but it did not happen again at the end of the application of SP (100%). The results of the SP1-SP4 action innovation in self-care deficit patients showed an increase in self-care ability. Patient I experienced the highest increase in ability 75% from 25% at the beginning of the study where the patient was able to explain the benefits of bathing, bath tools and materials, how to bathe, able to demonstrate bathing appropriately, and able to carry out daily activities after treatment considering the observation before treatment only able to explain the benefits of bathing. Recommendation: The provision of effective individual therapy implementation strategies to be implemented in nursing care for patients with self-care deficits
