Abstract

Background: Mothers of LBW babies with a deficit of knowledge about caring for LBW babies at home after returning from the hospital have a greater risk of providing care for LBW babies which allows LBW babies to experience problems in growth and development. Objective: To provide nursing care in the form of discharge planning for post partum mothers with a deficit of knowledge about caring for low birth weight babies at dr. Soedirman Kebumen. Method: applying the descriptive method through a case study approach. The subjects of the case study were five mothers of LBW babies with knowledge deficits about LBW care. The main study is the provision of education on the implementation of discharge planning at RSUD DR. Soedirman Kebumen. The research tools are in the form of nursing care formats, Nursing Kits, educational SAPs for LBW care, SAP for kangaroo care methods, evaluation sheets and observation sheets. Data collection uses interview, observation and documentation methods. The action was in the form of education on LBW care and the kangaroo method which was carried out for five days, with a frequency of four times. Results: The results of the study showed that the five respondents had a knowledge deficit about caring for LBW babies. The interventions were in the form of education on LBW baby care and the kangaroo method for mothers of LBW babies with knowledge deficits. The evaluation results show an increase in knowledge of LBW baby care and the results of observations show an increase in the emerging outcome criteria. Recommendation: The results of this nursing care can be used as a reference for preparing discharge planning SOPs for LBW mothers with knowledge deficits
