Abstract
Background: Diminished intracranial adaptive capacity refers to a disruption in the brain's dynamic mechanisms to compensate for stimuli that reduce its intracranial capacity. Effective management of post-craniotomy cases with intracerebral haemorrhage, involving reduced intracranial adaptive capacity, is essential to enhance patients' hemodynamic condition. Pharmacological therapy often involves providing oxygenation and positioning the patient in a head-up position. Objective: To elucidate the nursing care analysis for post-craniotomy intracerebral haemorrhage patients exhibiting reduced intracranial adaptive capacity. Methods: This study was a descriptive methodology, case study and analysed the administration of oxygenation and a 300-degree head-up position for patients displaying reduced intracranial adaptive capacity. The study took place within the Intensive Care Unit (ICU) of Prof. Dr. Margono Soekarjo Purwokerto Hospital. Results: The nursing problems identified among the five patients in this case study pertained to reduced intracranial adaptive capacity. The interventions employed encompassed pharmacological therapy adhering to recommendations and non-pharmacological methods, namely administering oxygenation and maintaining a head-up position of 300 degrees for 3x24 hours. Additionally, hemodynamic status was monitored before and after the procedure. Among the five patients, all exhibiting reduced intracranial adaptive capacity, a decline in consciousness with a Glasgow Coma Scale (GCS) score of ≤ 13 was noted. Post-procedure, an improvement in consciousness levels and enhanced hemodynamic status were observed. Recommendation: The outcomes of this study offer practical applications for post-craniotomy intracerebral hemorrhage patients grappling with reduced intracranial adaptive capacity. These findings can guide healthcare professionals in devising suitable interventions to optimize patient outcomes.
