Abstract
Background: Chronic Kidney Failure (CKD) patients undergoing hemodialysis often experience biopsychosocial-spiritual changes that lead to reduced self-esteem and social withdrawal. Comprehensive nursing care based on the concept of holism, as per Roy's adaptation model, aims to address these issues and provide holistic health services. Purpose: To report the outcomes of nursing care provided to patients with chronic kidney failure who experience situational low self-esteem. Method: This study utilizes a descriptive method with a case study approach involving five CKD patients. The assessment tool used is Rosenberg's self-esteem scale, measured both before and after a combination of generalist therapy and positive affirmation therapy. Positive affirmations are conducted in two sessions lasting 30 minutes each. Results: Nursing care for patients with chronic kidney failure and situational low self-esteem involved six sessions. The first four sessions focused on generalist therapy (SP 1-4), while the fifth and sixth sessions introduced a combination of positive affirmations. After generalist therapy, the five patients showed medium self-esteem, with Rosenberg's self-esteem scores ranging from 16 to 20. Following positive affirmation therapy, their self-esteem improved significantly, with scores between 26 and 28, indicating high self-esteem. The patients' abilities also increased after both types of therapy, with the highest improvement scores recorded as 4 (out of a range of 0-4) for generalist therapy and 5 (out of a range of 0-5) for positive affirmations. Recommendation: Healthcare professionals should consider applying generalist therapy and positive affirmation therapy to enhance self-esteem in chronic kidney failure patients in hospital settings. These holistic approaches can contribute significantly to improving the psychosocial well-being of CKD patients undergoing hemodialysis.
