Abstract

Background: According to WHO, the number of elderly people from year to year continues to increase along with the increase in life expectancy. One of the problems that often occurs in patients with hypertension is ineffective peripheral perfusion. Ineffective peripheral perfusion is a decrease in blood circulation at the capillary level which can interfere with the body's metabolism. One of the non-pharmacological therapies that can be done is therapy by soaking the feet using warm water to lower blood pressure. Objectives: Explaining the analysis of gerontic nursing care with the main nursing problem, peripheral perfusion is not effective with the application of warm water foot baths. Methods: This research uses descriptive case study method. The subjects of this study were elderly patients with hypertension according to the criteria. Result: Nursing diagnosis is ineffective peripheral perfusion. Interventions: SLKI decreased pulse rate, increased skin color, improved capillary refill, improved systolic blood pressure, improved diastolic blood pressure. SIKI circulation treatment by checking peripheral circulation, monitoring vital signs, providing hydration, monitoring the success of giving warm water foot bath therapy. The implementation carried out by the five patients was warm water foot soak therapy in hypertensive patients, by following the procedure according to the applicable SOP, with a duration of 10 minutes each day for 1x a day for 3 consecutive days. For the evaluation, based on the five patients who received warm water foot bath therapy, they experienced a significant decrease in BP. The average blood pressure before the procedure was 165/105 after 145/90, and the average pale skin color was not pale and cold extremities became warm. Conclusion: There is an effect of giving warm water foot bath therapy on peripheral perfusion which is not effective in hypertensive patients.
