Abstract
Background : Medication errors can occur at four stages (prescribing, prescription translation, preparing and compounding the medication, and administering the medication to the patient). Errors during the prescribing stage usually occur primarily due to the selection of the medication.The prescribing stage involves reviewing the prescription to reduce or minimize medication errors,and such errors can be decreased through the increased use of pharmacy management tools, suchas the implementation of electronic prescriptions. Objectives : To determine the effect of changes in prescribing models from the use of non- electronic prescriptions to electronic prescriptions on prescribing errors in the internal medicineclinic at PKU Muhammadiyah Gombong Hospital. Methods : The research method used is a quantitative approach with a retrospective research type. The sample used was 390 recipes with random sampling. Result : This research shows that the application of electronic prescriptions has a significant effecton prescribing errors, completeness of prescriptions (p value 0.000<0.05 and the application of electronic prescriptions reduces the incidence of prescribing errors (prescription completeness, drug interactions and drug duplication) compared to non-electronic prescriptions. Conclusion : The implementation of electronic prescriptions reduces the incidence of prescribing errors (prescription incompleteness, drug interactions and duplication) and has a significant influence on prescription completeness (p value 0.000<0.05). Recommendation : Research recommendations given in accordance with the results of the study include future researchers can carry out comparative analysis of medication errors at other stages, namely the transcribing and dispensing stages.
