Abstract

Background: Congestive heart failure is a chronic condition with a high mortality rate in Asia, including Indonesia. Optimal management by nurses, particularly using the Semi-Fowler position, has proven effective in improving oxygen saturation and patient quality of life. Objective: To analyze nursing care by applying the Semi-Fowler position for congestive heart failure patients with ineffective breathing patterns in the ICCU of Prof. Dr. Margono Soekarjo Hospital, Purwokerto. Method: This study used a case report method with 5 congestive heart failure patients experiencing ineffective breathing patterns as subjects. Tools included nursing care formats, a nursing kit, observations of breathing pattern symptoms, and the Semi-Fowler position SOP. Data were collected through subjective and objective data conclusions, documented in nursing care summaries. Results: Congestive heart failure patients primarily complained of shortness of breath. The priority nursing diagnosis was ineffective breathing pattern. Interventions included airway management and the Semi-Fowler position. Implementation involved airway management, monitoring of breathing patterns and sounds, and therapeutic actions to maintain airway patency and patient positioning. Evaluation indicated resolution of the ineffective breathing pattern issue, evidenced by a decrease in respiratory rate (RR) and an increase in oxygen saturation (SpO2). The Semi-Fowler position innovation proved effective, with all patients showing decreased RR, increased SpO2, and no additional breath sounds. Conclusion: The Semi-Fowler position is effective in addressing ineffective breathing patterns in congestive heart failure patients in the ICCU, as evidenced by reduced respiratory rate and increased oxygen saturation. Recommendation: The Semi-Fowler position should be routinely applied by nurses in managing congestive heart failure patients with respiratory issues.
