Abstract
Background: Acute respiratory tract infections (ARTIs) remain a significant public health problem in Indonesia, exhibiting a high prevalence, especially among children. Irrational antibiotic use in the treatment of ARTIs poses serious threats, such as antimicrobial resistance and adverse drug reactions. Objective: This study aimed to evaluate the rationality of antibiotic use in patients with upper acute respiratory tract infections at Rowokele Community Health Center. Methods: This retrospective descriptive study used purposive sampling to select 100 patient medical records from Rowokele Community Health Center. Data were analyzed using descriptive statistics, calculating frequencies and percentages for patient characteristics, antibiotic types, and the appropriateness of each aspect of antibiotic use (indication, drug, dose, route, duration, and interval). Results: Most patients with ARTIs at Rowokele Community Health Center received single-agent antibiotic therapy, predominantly amoxicillin (125–500 mg) and cefadroxil (500 mg). Antibiotic use was appropriate with regard to patient factors, indications, dosage, drug selection, and route of administration. However, discrepancies were found in treatment duration (43%) and dosing intervals (6%). Overall, only 55% of antibiotic prescriptions were considered rational. Conclusion: This evaluation revealed that while antibiotic prescribing for ARTIs at Rowokele Community Health Center was largely appropriate concerning the selection of antibiotics, significant discrepancies remain in treatment duration and dosing intervals. This highlights the need for enhanced monitoring and educational interventions to promote rational antibiotic use. Recommendations: Educational interventions targeting both patients and healthcare providers on rational antibiotic use are recommended. Furthermore, closer monitoring of treatment duration and dosing intervals is crucial to minimize misuse and optimize treatment outcomes
