Abstract
Background: According to the analysis of maternal deaths in 2014 to the latest data obtained from the Indonesian Demographic and Health Survey (IDHS) in 2012, there is a surprising fact about maternal and infant mortality. IDHS results maternal mortality ratio (MMR) had reached 359 per 100,000 live births. This average is much increased compared to 2007, which recorded the results IDHS lift 228 per 100 thousand live births. It becomes ironic because the target of the Millennium Development Goals (MDG's) of Indonesia in 2015 alone was 108 per 100 thousand live births, mortality may increase due to lack of attention in implementation aspects of Maternal and Child Health (MCH) area in the context of decentralization. Objective: to apply the theory that has been obtained in institutions with a comprehensive midwifery care to pregnancy, childbirth, newborn, family planning and Integrated Management of Young infant (ind=MTBM) in the field using SOAP documentation to know whether there is a gap between theory and practice fields. Methods: This is a descriptive qualitative research with a case study approach. The subject is Mrs. R, age 23 years with her 2nd pregnancy and she had not had any experience of miscarriage. This study was conducted from 16 April 2014 until June 10, 2014. Results: Based on a comprehensive assessment ranging from pregnancy, labor, childbirth, newborn, family planning and the Integrated Management of Young Infant, the results were normal with no data or pathological leading emergency and there is no gap between theory and practice fields. Conclusion: it can be concluded that the comprehensive midwifery care obtain physiological results. There are some gaps between theory and practice.
