Abstract
Background: hemorrhagic stroke is a rupture of blood vessels in the brain, so occur ischemic and hypoxic. In general hemorrhagic stroke be found edema cerebral can be increase intracranial pressure. Purpose: Describes nursing care of hemorrhagic stroke patients with ineffective perfusion of the cerebral tissue: intracranial pressure monitors by 300 heads up positioning a comprehensive. Nursing care: assessment obtained was loss of consciousness with GCS E2V2M4, the authors found ineffective perfusion of the cerebral tissue correlated with bleeding in the brain cavity and a nursing plan that head up 300 positioned to reduce intracranial pressure, because as the patient had loss consciousness. The authors assess clients with triad crushing to know that clients experience increased intracranial pressure, consciousness monitor, vital sign monitor, and therapy according to instructions. The planned action has been done by the author of 3x24 hours with the evaluation result in effective perfusion problems of cerebral tissue related to bleeding in the brain resolved. The focus of action: Head-up action effective to control the increase intracranial pressure.
