Abstract
Background: The number of chronic diseases experienced by prolanis participants are Hypertension and Diabetes Mellitus. The incidence increases in 2018 as many as 8.5% of patients with diabetes mellitus, and the incidence increases by 34.1% of patients with hypertension. Prolanis is a system of health services and a proactive approach that is implemented in an integrated manner to maintain health for participants suffering from chronic diseases to achieve optimal quality of life with the cost of effective and efficient health services (BPJS Kesehatan 2018). Objectives:Finding out the implementation of the Chronic Disease Management Program (PROLANIS) activities at Ayah health center 1. Method: This study is a quantitative descriptive study, the population in the study was 72 respondents, the sample used a saturated sample (population representing samples) of 72 samples. Results: The results of the participants' activeness research in carrying out prolanis activities were from medical consultation programs (58.3%), education (58.3%), sms reminder gateways (58.3%), home visits (7%), gymnastics ( 58.3%), blood pressure checks (36.1%), blood sugar levels (36.1%), penkes (58.3%), and health screening (63.9%). Conclusion: The study showed that the most characteristic of respondents were female sex (70.9%), occurred at the age of 46-65 years (63.9%), elementary education (50%) and respondents did not work (44.4%). Some participants experienced hypertension as many as 26 participants and blood sugar levels were abnormal or exceeded the normal limit of 46 participants. Recommendation, This study is recommended to use a type of qualitative research, so that will know more deeply about the implementation of PROLANIS.
