Abstract
Background: Breast milk (ASI) is the best source of nutrition for newborns, yet ineffective breastfeeding remains a common issue among postpartum mothers. One contributing factor is the lack of stimulation of the hormones oxytocin and prolactin. Oketani massage is a breast massage technique proven to increase milk production. Objective: This study aims to describe nursing care for spontaneous postpartum mothers experiencing ineffective breastfeeding by implementing the Oketani massage technique. Method: This study used a descriptive case study method on three postpartum clients who experienced ineffective breastfeeding. The nursing interventions carried out were Oketani massage for 3 consecutive days, twice a day, each for 10-15 minutes. Data were collected through interviews, observations, and documentation. Results: Before receiving Oketani massage, all three clients showed low breast milk production (score 1–2). After the intervention, their milk production scores improved to an average of 4–5 on the third day, indicating sufficient milk output. In addition, clients reported a reduction in perineal pain, with pain levels decreasing from a scale of 6 to 2. Recommendation: Oketani massage is effective in increasing breast milk production and reducing postpartum perineal pain. This technique can be considered a non pharmacological nursing intervention in maternity care
