Abstract

Background: Urinary Tract Infection (UTI) is one of the most common infectious diseases in Indonesia, with a high prevalence, particularly in Central Java at 13.5%. Antibiotic therapy is the main treatment; however, resistance and clinical effectiveness must be regularly evaluated. Evaluating antibiotic effectiveness is crucial to ensure therapeutic success, prevent complications, and minimize the risk of ongoing resistance. Objective: To assess the comparative effectiveness of antibiotic use in inpatients with UTI at Permata Medika Hospital Kebumen. Methods: This research is a non-experimental observational study with a retrospective approach. Data were collected from medical records of 96 inpatient UTI patients from January 2022 to December 2024, of which 77 patients met the inclusion criteria, while 19 were excluded. Result: The most commonly used first-line antibiotic was Ceftriaxone, administered to 60 patients (77.92%), and the second-line antibiotic was Gentamicin, used in 2 patients (2.60%). The analysis showed that both first- and second-line antibiotics were effective in reducing leukocyte counts in 75 patients (97.40%), lowering body temperature in 77 patients (100%), and shortening the length of hospitalization in 52 patients (67.50%). Statistical tests indicated no significant differences in the effectiveness of first- and second-line antibiotics in terms of leukocyte count, duration of hospitalization, and neutrophil levels (Pvalue > 0.05). Conclusion, Both first- and second-line antibiotics were equally effective. Recommendation: Further research is needed with a larger sample size, as well as bacterial culture and sensitivity testing.
