Abstract

Background: In patients with chronic kidney disease (CKD), appropriate dosage adjustment is essential to avoid adverse drug effects and to ensure optimal therapeutic outcomes, thereby achieving effective treatment. Objectives: To determine the drug dosage patterns in CKD patients and their relationship with therapeutic outcomes in the inpatient unit of Dr. Soedirman Regional General Hospital Kebumen in the period 2024. Methods: This study used a descriptive-analytical design with a cross-sectional approach. Data were collected retrospectively using total sampling of 52 patients. Bivariate analysis was conducted to examine the relationship between independent and dependent variables using the chi-square test. Results: Of the 52 patients, the majority were male (27 patients, 51.92%), aged 51–59 years (24 patients, 46.15%), in stage 4 CKD (42 patients, 80.76%), and had anemia as a comorbidity (22 patients, 30.77%). A total of 133 drugs required dose adjustment, with 104 prescriptions (78%) properly adjusted, while 29 prescriptions (22%) were not adjusted. Therapeutic outcomes improved in 28 patients (53.84%), while 24 patients (46.15%) showed no improvement. The analysis of the relationship between dosage adjustment and therapeutic outcomes showed a significant correlation, with a p-value of 0.015, indicating that H0 was rejected and H1 was accepted. Conclusion: Drug dosage adjustment has a significant relationship with improved therapeutic outcomes in patients with chronic kidney disease, with 28 patients (53.84%) experiencing improvement. Recommendation: Further research is recommended to use prospective methods for more comprehensive results.
