Abstract
Background: The prevalence of ischemic stroke with hypertension continues to increase each year. Blood pressure control is essential to prevent recurrence and complications. At Dr. Soedirman Regional General Hospital Kebumen, there is variation in the use of single antihypertensive agents. Amlodipine and candesartan are frequently prescribed due to their effectiveness and relatively mild side effect profiles. Therefore, an analysis of the effectiveness of both therapies is needed to support the selection of the optimal regimen. Objective: To compare the effectiveness of single antihypertensive drugs from the ARB and CCB classes in ischemic stroke patients in the inpatient ward of Dr. Soedirman Regional General Hospital Kebumen in 2024. Methods: This research is a descriptive analytic study with a cross-sectional design using retrospective data from medical records of ischemic stroke patients in 2024. Sampling was conducted using total sampling, with a total of 112 patients meeting the inclusion criteria. Bivariate analysis was used to assess the relationship between various factors and therapy effectiveness using the Chi-Square and Mann-Whitney tests. Results: Based on age characteristics, 40 patients (35.71%) were <65 years old, and 72 patients (64.29%) were >65 years old. Gender distribution showed 55 female patients (49.11%) and 57 male patients (50.89%). Both therapies effectively reduced blood pressure to target levels (<140/90 mmHg). Statistical tests showed no significant difference between the two therapies (p = 0.995; p > 0.05). Conclusion: Both ARB and CCB drug classes are equally effective in lowering blood pressure in ischemic stroke patients without significant differences. Recommendation: Further studies with a prospective design are needed to evaluate side effects, quality of life, and pharmacoeconomics aspects to assess cost-effectiveness
