Abstract

Background, Major thalassemia patients need iron chelation to eliminate excess iron from transfusions. However, adherence is often low due to difficulties with deferoxamine (DFO) use, drug intolerance, psychological factors, and side effects like dizziness, hearing loss, and growth delays. Therefore, analyzing adherence to chelation therapy is crucial. Research Objective, This study aims to analyze the level of compliance in the use of iron chelation therapy and the factors influencing it in major thalassemia patients DR. Soedirman Hospital Kebumen. Research Method, This study uses an analytical observational method with a crosssectional approach. Prospective data collection on major thalassemia patients will begin during the period of March–April 2025 according to the inclusion criteria. The level of therapy adherence was measured using the MMAS-8 scale, and the relationship between variables was analyzed using the chi-square test. Research Result, The study on 74 major thalassemia patients at DR. Soedirman Hospital Kebumen in 2025 found that 62.2% were adherent and 37.8% nonadherent to iron chelation therapy. Factors significantly influencing adherence included gender, distance from residence, and remaining medication. Meanwhile, age, education level, health insurance status, parental employment and income, family support, and duration of illness showed no significant effect Conclusion, Analysis shows that the majority of major thalassemia patients at DR. Soedirman Hospital (62.2%) have low adherence to iron chelation therapy. Recommendation, Further research is needed on the analysis of compliance with clinical outcomes to provide an overview of the quality of life of patients with major thalassemia.
