Abstract

Background: Pneumonia is a respiratory infection that often leads to ineffective airway clearance due to increased secretion production. This exacerbates gas exchange and increases the risk of complications such as dyspnea and hypoxia. The Active Cycle of Breathing Technique (ACBT) has been shown to effectively reduce sputum production, improve respiratory efficiency, and assist in clearing airways in pneumonia patients. Objective: This study aimed to analyze nursing care for pneumonia patients with ineffective airway clearance at PKU Muhammadiyah Kutowinangun Hospital through the implementation of ACBT as an innovative nursing intervention. Methods: This research used a descriptive method with a case study approach. The subjects were five pneumonia patients experiencing ineffective airway clearance. Instruments included a nursing care format, Nursing Kit, and Standard Operating Procedure (SOP) for implementing ACBT. Data were analyzed qualitatively and presented in tabular and narrative forms. Results: Based on the assessment, all five patients exhibited primary symptoms such as shortness of breath, productive cough, and secretion accumulation. Data analysis identified the nursing diagnosis of ineffective airway clearance related to increased secretion production. Nursing interventions included monitoring vital signs, semi-Fowler positioning, oxygen administration, effective coughing technique education, and ACBT implementation. Evaluation after the intervention showed significant improvements: a decrease in respiratory rate by an average of 2-3 breaths per minute, an increase in oxygen saturation by 1-2%, thinner sputum consistency, and increased sputum expectoration. Conclusion: The implementation of ACBT was effective in addressing ineffective airway clearance in pneumonia patients. This technique can be integrated into standard nursing care to improve patient quality of life. Recommendation: ACBT can become a routine nursing intervention for pneumonia patients with ineffective airway clearance.
