Abstract

Background: WHO estimated that 1.28 billion adults aged 30-79 years have hypertension. According to Riskesdas, the hypertension rate in Indonesia is 33%. Hypertension still occupies the largest proportion of all noncommunicable diseases reported in Central Java. Elderly is an irreversible risk factor for hypertension. Some symptoms that may appear include head pain, dizziness, and fatigue. The combination of Benson relaxation and warm water foot soaking as hypertension management in the elderly is proven to reduce pain. Objective: This research is a descriptive qualitative case study of the nursing care process in elderly hypertension with chronic pain. This study involved five hypertensive elderly with pain for more than 3 months. The research was conducted in Gumelar Kidul Village from February 18th to March 8th 2025. Methods: Methods are starting from assessment, diagnosis, intervention, implementation, evaluation using gerontic nursing care format and CNPI questionnaire. The implementation of benson relaxation with warm water foot soaking hydrotherapy was given three times per week, for three weeks. Results: The application of benson relaxation technique and warm water foot soaking hydrotherapy showed a significant decrease in CNPI score from 4-6 to 1-2. This technique could reduce pain, systolic and diastolic blood pressure, which is in accordance with previous studies that showed the effectiveness of benson relaxation technique and warm water foot soaking hydrotherapy in reducing pain and lowering blood pressure. Recommendation: The results of this study can be a safe nonpharmacologic strategy to help manage chronic pain in hypertension elderly.
