Abstract

Background: A Caesarean Section (CS) is often necessary when expected delivery is not feasible, but it frequently leads to lactation challenges for postpartum mothers. In Indonesia, the C.S. rate reached 17.6% in 2018—difficulties in breastfeeding after C.S. can negatively affect both the mother and the baby. Woolwich massage, known for its benefits in promoting breast milk production, has proven effective in overcoming lactation challenges post-CS, making it a critical intervention for mothers after surgery. Objective: This study aims to analyze nursing care for postpartum mothers experiencing ineffective breastfeeding following a Caesarean section in the Flamboyan Ward at RSUD. Prof. Dr. Margono Soekarjo Purwokerto. Methods: This study employed a descriptive method with a case study approach. The subjects were five postpartum mothers with ineffective breastfeeding following a Caesarean section. Instruments included a nursing care format and the Woolwich Massage Standard Operating Procedure (S.O.P.). Data analysis was performed descriptively and qualitatively, and it was presented in tables and narratives. Results: Assessments of Clients I-V revealed ineffective breastfeeding, characterized by complaints of insufficient breast milk production, lack of breastfeeding experience, hard breasts, protruding nipples, and fussy babies. The primary nursing diagnosis for all clients was ineffective breastfeeding. Interventions included assessing breast milk output, educating mothers on proper breastfeeding techniques and breast massage, monitoring breast milk flow, encouraging feeding from both breasts during each session, and promoting increased breastfeeding intensity using Woolwich massage. The nursing care provided as planned was successful, and evaluation results indicated that the issue of ineffective breastfeeding was resolved in all five clients. The nursing intervention significantly reduced symptoms of ineffective breastfeeding and increased adequate breast milk production in Clients I-V. Conclusion: Nursing care involving Woolwich massage effectively addressed ineffective breastfeeding in postpartum mothers following a Caesarean section at R.S.U.D. Prof. Dr. Margono Soekarjo Purwokerto, enhancing breast milk production and supporting maternal recovery. Recommendation: Health professionals are encouraged to routinely incorporate Woolwich massage into the care of postpartum mothers after a Caesarean section to improve breastfeeding success.
