Abstract

Background: Pulmonary tuberculosis (TB) is a respiratory tract infectious disease that often causes secretion accumulation in the lungs, potentially leading to ineffective airway clearance. This condition can exacerbate symptoms such as productive cough, dyspnea, and hypoxemia, negatively affecting patients’ quality of life and prolonging hospitalization. Chest physiotherapy, as part of nursing care, plays a crucial role in facilitating secretion removal and improving respiratory function. Objective: This case study aims to analyze the implementation of nursing care in managing ineffective airway clearance in pulmonary TB patients using a combination of conventional management and innovative chest physiotherapy interventions. Methods: This study employed a qualitative descriptive case study design. The subjects consisted of five pulmonary TB patients hospitalized in the Flamboyan Ward of Prembun District Hospital. Data were collected through interviews, observation, physical examination, and medical documentation. Nursing interventions were implemented based on the Indonesian Nursing Intervention Standard (SIKI) I.01011, including routine observation, airway management (semi-Fowler position, warm fluid intake, oxygenation), chest physiotherapy (postural drainage, percussion, vibration, deep breathing exercises, and controlled coughing), and education on effective coughing techniques. Evaluation was conducted formatively and summatively using the SOAP format. Results: Following the implementation of nursing care, all patients showed improvement, indicated by reduced cough frequency, increased oxygen saturation (SpO₂ ≥92%), absence or minimal ronchi on auscultation, and improved breathing patterns. Although mild cough persisted, the problem of ineffective airway clearance began to resolve. Conclusion: Comprehensive nursing care, particularly with the integration of chest physiotherapy, proved effective in improving airway clearance in pulmonary TB patients. Regular monitoring, patient education, and interdisciplinary collaboration are essential in supporting therapeutic success.
