Abstract

Background: Diabetes mellitus (DM) is a health condition caused by impaired glucose metabolism, accompanied by various metabolic abnormalities due to hormonal problems. Helplessness is the client's perception or reaction that their behavior or actions will not produce the desired results or will not change the outcome as desired. Psychosocial support can be provided through general therapy, one of which is positive affirmation therapy. Objective: To analyze nursing care for helplessness in diabetes mellitus patients with hope promotion actions: positive affirmations at PKU Muhammadiyah Gombong Hospital. Method: This case study used 5 respondents, consisting of diabetes mellitus patients treated in the Barokah Room of PKU Muhammadiyah Gombong Hospital who experienced helplessness using the Beck Hopelessness Scale (BHS) questionnaire. The implementation consisted of a hope promotion action: positive affirmations given for 15 minutes for 3 days and analyzing the helplessness scores before and after the positive affirmations were given. Results: The study results showed that the patient who experienced the highest reduction in signs and symptoms of helplessness was P4, with a score decreasing from 31 at pre-intervention to 24 at post-intervention, thus experiencing a decrease of 7 points. The patient with the highest reduction in score was P4, who experienced a decrease in helplessness score from 17 (severe category) at pre-intervention to 9 (moderate category) at post-intervention, with a difference of a decrease of 8 points. All patients experienced a decrease in helplessness scores at each meeting with an average decrease of 5 points after therapy was given. Patients appeared more confident, began to accept their condition, and were able to express feelings positively. Conclusion: The majority of respondents at PKU Muhammadiyah Gombong Hospital experienced a decrease in their powerlessness scores and an increase in their ability to perform positive affirmations. Recommendation: Positive affirmations can be applied because they are effective in reducing helplessness and helping patients increase self-acceptance.
