Abstract

Background: Congestive heart failure (CHF) is a leading cause of global mortality, with significant incidence rates in Indonesia, particularly in Kebumen. Decreased cardiac output in CHF patients leads to symptoms like fatigue and shortness of breath. Home-Based Exercise Training (HBET) has been proven effective in improving patients' quality of life. Objective: This study aims to analyze the nursing care implementation of HBET in congestive heart failure patients with decreased cardiac output at RSUD Dr. Soedirman Kebumen. Method: This descriptive study uses a case study approach. The subjects were five CHF patients with decreased cardiac output. Instruments used included nursing care formats, Nursing Kits, and SOPs for the innovative HBET intervention. Data were analyzed descriptively using qualitative methods and presented in tables and narratives. Results: Assessment showed that all five CHF patients experienced worsening shortness of breath during activity. The nursing diagnosis indicated decreased cardiac output related to changes in heart rhythm, preload, and afterload. Interventions included monitoring vital signs, ensuring patient comfort, providing relaxation therapy, and educating patients on gradual physical activity. Implementing HBET effectively reduced shortness of breath, tachycardia, and increased oxygen saturation, proving it to be an effective innovation in CHF management. Conclusion: The implementation of HBET in congestive heart failure patients at RSUD Dr. Soedirman Kebumen effectively reduces shortness of breath, tachycardia, and increases oxygen saturation, thereby improving patient quality of life. Recommendation: HBET should be integrated into CHF nursing care, with training for nurses and education for patients and families to ensure its effectiveness.
