Abstract
Background: Hypertension is a chronic disease that requires long-term therapy and patient adherence to antihypertensive medication to prevent complications. Non-adherence to therapy may lead to uncontrolled blood pressure and a decreased quality of life. However, the relationship between medication adherence and the quality of life among hypertensive patients at RS PKU Muhammadiyah Gombong remains unclear. Objective: The aim of this study was to analyze the relationship between adherence to antihypertensive therapy and the quality of life among hypertensive patients at RS PKU Muhammadiyah Gombong. Methods: This study employed a descriptive correlational design with a cross-sectional approach. The sample consisted of 95 hypertensive patients who met the inclusion criteria, namely aged >25 years, diagnosed with hypertension with or without comorbidities, and undergoing antihypertensive therapy for at least the past one month. Data were collected using the MMAS-8 and WHOQOL-BREF questionnaires. The relationship between variables was analyzed using Fisher’s Exact test. Results: The results of this study showed that the majority of patients had a high level of adherence, with 54 patients (56.8%) in this category, followed by moderate adherence with 24 patients (25.3%) and low adherence with 17 patients (17.9%). In the physical domain, most patients were in the moderate category (77 patients; 81.05%). The psychological domain was dominated by the good category (59 patients; 62.11%). In the social domain, most patients were in the good category (58 patients; 61.05%), followed by the very good category (26 patients; 27.37%). Meanwhile, the environmental domain was predominantly in the moderate category (79 patients; 83.16%). The results of Fisher’s Exact test indicated that there was no significant relationship between medication adherence and the quality of life of hypertensive patients (p > 0.05). Conclusion: This study found no significant relationship between medication adherence and the quality of life among hypertensive patients at RS PKU Muhammadiyah Gombong (p > 0.05). These findings suggest that the quality of life in hypertensive patients is a multidimensional construct influenced by physical, psychological, social, and environmental factors. Recommendation: Further studies are recommended to employ multivariate analysis to better identify factors associated with medication adherence and the quality of life among hypertensive patients.
