Abstract
Background: Schizophrenia isasevereneuropsychiatricdiseasethatoftenappearsinlate adolescence toearlyadulthoodandischaracterizedbyabroadspectrumofsymptoms including hallucinations.Auditoryhallucinationsarethemainpositivesymptomcaused by afailureofsignalintegrationinthebrain,manifestingasaninternalauditory experience withoutarealexternalsoundsource.Thisdisordercausesgreatpersonaland family sufferingaswellasasignificantdeclineinfunctionalandsocialfunction. General Objective: To explainthenursingcareforaclientwithundifferentiated schizophrenia whosemainproblemissensoryperceptiondisorder:auditory hallucinations intheSerojaWardofRSUDdr.SoedirmanKebumen. Method: This reportusesadescriptivecasestudydesignonMr.R(age14years8 months) withamedicaldiagnosisofUndifferentiatedSchizophrenia.Thenursingcare approach wascarriedoutbyreferringtotheIndonesianNursingDiagnosisStandards (SDKI), IndonesianNursingOutcomesStandards(SLKI),andIndonesianNursing Intervention Standards(SIKI).Thecarewasimplementedduringthetreatmentperiodof November 9–13,2025. Conclusion: The resultsofthenursingcareshowedthatafterassessmentandevaluation, the clientdemonstratedsignificantprogress.Implementedinterventionsincludeda scolding technique(tocounteracthallucinations),conversationpractice,schedulingdaily activities, andmedicationadherenceeducation.ThefinalevaluationonNovember13, 2025, showedthatthenursingproblemofSensoryPerceptionDisorder:Auditory Hallucinations wascompletelyresolved,asindicatedbythecessationofhallucinations and theclient'sbehaviorbeingcongruentwithreality. Recommendations: The hospitalinstitutionisexpectedtooptimizetheprovisionof structured psychosocialinterventionsandstrengthenprotocolsformonitoringmedication side effects.Futureresearchersarerecommendedtoexploretheeffectivenessof combining GeneralistPsychotherapywithpharmacotherapytoimproveclients' receptiveness tohallucinationcontroltechniques,aswellastostudytheeffectivenessof family educationprogramsinsupportinglong-termtreatmentadherence
