Abstract

Background: Indonesia has the highest stroke mortality rate in Southeast Asia. Cognitive impairment is one of the symptoms of stroke, which involves the ability to receive, process, store, and reuse all sensory input effectively. Cognitive impairment leads to a decreased ability to perceive reality, resulting in an inability to recognize people, places, and times. Reality orientation therapy can be administered appropriately. Research Objectives: To implement the Nursing Care Process for Non-Hemorrhagic Stroke Patients with Mild Cognitive Disorders Using Reality Orientation Therapy Interventions at PKU Muhammadiyah Gombong Hospital. Research Methods: This study used a descriptive case study design in five patients with non-hemorrhagic stroke who experienced mild cognitive impairment. The nursing care process was carried out through the stages of assessment, diagnosis, intervention, implementation, and evaluation. The main intervention was reality orientation therapy. Measurements were conducted using the MMSE scale before and after the intervention to assess the cognitive level of the respondents. Research Results: The assessment results showed that all five patients experienced orientation disorders, such as not knowing their condition and forgetting their names and those of some family members. The primary nursing diagnosis was memory impairment. The evaluation showed an increase in cognitive levels before and after 3 x 24 hours of therapy. On average, patients experienced a cognitive improvement of 2-3 points using the MMSE measurement tool. Conclusion: Reality orientation therapy has an impact on improving cognitive function in non-hemorrhagic stroke patients with mild cognitive impairment. Recommendation: Nurses are expected to be able to apply reality orientation therapy to improve memory.
