Abstract
Background: Typhoid fever is a systemic infectious disease that remains a major health problem in developing countries, including Indonesia, with a high prevalence among preschool-aged children. Children with typhoid fever are prone to anxiety, particularly during intravenous (IV) bolus injection procedures. Unmanaged anxiety may hinder the treatment process. One non-pharmacological intervention is the bear hug position therapy, which provides a sense of security and comfort. Objective: To implement nursing care for children with typhoid fever experiencing anxiety during intravenous (IV) bolus injections through the application of bear hug position therapy. Methods: This study used a descriptive case study approach involving five pediatric patients aged 2–5 years. Data were collected through the nursing care process, including assessment, diagnosis, intervention, implementation, and evaluation. Anxiety levels were measured using the Facial Image Scale (FIS) before and after the intervention over a period of four days. Results: All patients experienced a reduction in anxiety levels after receiving bear hug position therapy. The average anxiety score decreased from 4.4 to 2.8, indicating a shift from moderate–severe anxiety to mild anxiety. The reduction occurred gradually in each patient. Recommendation: Nurses are encouraged to apply bear hug position therapy as a non-pharmacological intervention to reduce anxiety. Future studies are recommended to examine the effectiveness of this therapy on other variables such as pain.
