Abstract

Background: Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) is a chronic disease that attacks the immune system and remains a global and national public health problem. The success of antiretroviral (ARV) therapy is highly influenced by patients’ adherence to medication, which can be evaluated through viral load examination as an indicator of virological suppression. Objectives: This study aimed to determine the relationship between adherence to antiretroviral therapy and viral load among HIV/AIDS patients at the outpatient clinic of RSUD dr. Soedirman Kebumen. Methods: This study was an analytic observational study with a cross-sectional design. The sample was obtained using purposive sampling, consisting of 81 respondents out of 128 respondents who were selected based on inclusion and exclusion criteria, including HIV/AIDS patients undergoing ARV therapy, having viral load data within the last 6 months, and not having severe comorbidities or active infections. Adherence was measured using the MMAS-8 questionnaire, while viral load data were obtained from medical records and the HIV/AIDS Information System (SIHA). Data were analyzed using the Chi-Square test. Results: Most respondents had moderate adherence (44.4%), followed by high (42%) and low adherence (13.6%). The majority of respondents were in stable condition with suppressed viral load (<1000 copies/ml) at 98.8%, while 1.2% were unsuppressed (≥1000 copies/ml). The Chi-Square test showed no significant relationship between adherence to antiretroviral therapy and viral load (p>0.05). Conclusion: There was no significant relationship between adherence to antiretroviral therapy and viral load. Recommendation: Future studies are recommended to use more representative samples and more objective adherence measurement instruments to obtain more valid and generalizable results.
