Abstract
Background: Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain a global concern, including in Indonesia, which has the third highest MMR among ASEAN countries. In Kebumen Regency, the MMR was also still high in 2024, with 15 maternal deaths. Maternal and infant deaths can be avoided if problems are identified early. Continuous support is essential to ensure early detection of all risks. General Objective: To provide continuous midwifery care at PMB Dwi Widiyati., Amd. Keb. Methods: This study used a case study method with a SOAP midwifery care approach. Data were obtained through interviews, observations, examinations, complementary midwifery care, and documentation. Care was provided at health care facilities and at the respondents' homes. Results: After midwifery care, the respondents experienced moderate anemia during their pregnancy, necessitating a hospital examination. Delivery occurred spontaneously with recommended induction at 39+2 weeks of gestation, with both mother and baby in good health. The postpartum period was physiological, and the respondents chose the mini-pill as a contraceptive method. Conclusion: Implementing continuous midwifery care provides benefits because it allows for early detection of problems, thus avoiding potential risks, thus improving maternal and child health. It is recommended that midwives continue to provide continuous midwifery care to improve the quality of care and improve the well-being of mothers and children.
