Abstract

Background: Continuity of Care is a midwifery care approach provided continuously from pregnancy, through labor and delivery, the neonatal period, the postpartum period, to family planning services. This approach aims to improve the health of mothers and infants, prevent complications, and provide optimal emotional support General Objective: To provide continuous midwifery care to Ms. S, a G2P0A1 patient, from 35+5 weeks of gestation through 14 days postpartum, in accordance with midwifery care standards, to monitor the health of the mother and infant and support the mother’s role adaptation Methods: This study employed a case study design using Varney’s 7-step midwifery management approach as a problem-solving framework, which was then documented using the SOAP (Subjective, Objective, Assessment, Planning) method. Data were collected through interviews, observations, physical examinations, and review of medical records. Care was provided directly at the patient’s home and at a health care facility. Outcome of midwifery care: During the period of midwifery care, the pregnancy progressed normally; however, at 40+2 weeks’ gestation, the patient consulted a specialist who diagnosed suspected cephalic malpresentation and umbilical cord entanglement, so the patient was advised to deliver via cesarean section at the hospital. Delivery occurred via cesarean section at 40+2 weeks of gestation, with both the mother and baby in good health. The postpartum period proceeded normally, and the mother chose an implant for birth control after the postpartum period ended. Recommendation: The implementation of Continuity of Care provides significant benefits in the early detection of problems, increasing the mother’s knowledge, and achieving optimal health for both mother and baby. It is recommended that midwives continue to implement Continuity of Care as part of routine care to improve the quality of midwifery services.
